
Our Outreach Worker will get to know young Sickle Cell clients and their families and act as a permanent link between the health professionals and the clients to make sure they are taking medication, keeping regular appointments, that their education is not suffering because of the Sickle Cell constraints etc. Any money raised will be used to provide funds for a regular user group so that peers can support each other now and in the future
Why they need our serviceWhilst children suffering from Sickle Cell Disorder are living with their parents, they rely on them for their specific health needs. However as they get older they should achieve a level of independence and control over their own lives and health. OSCAR has identified a need for a transition service to support individuals aged from 13 - 21 during this difficult transition. This will include helping them secure and maintain college places, or jobs, changing hospitals, relieving isolation.
Plan and expected resultsIf young clients take care of their health in this way, they will suffer fewer crises and relieve pressure on the NHS through fewer hospitalisations and broken appointments. They will also benefit in later life by taking control of their own health and understanding more about living a fuller more rewarding life.
