Transitions trains volunteer mentors and then matches them people who are making that transition to independent living. The aim is to increase their resilience through developing their skills and building networks in their communities. Their mentor meets them regularly and offers a range of support from household budgeting, food shopping to job-hunting or joining clubs in their area.

Statutory services for people who need extra support are often time limited. Transitions mentors are there to help people cross that final bridge to independence. They aim is to ensure people make that transition to independent living with slipping backwards into the system. They offer help with practical matters and as well offering social support.

We work with over 50 people per year, helping them to improve their lives and maintain their independence.
