Paul Sartori Hospice at Home's aim is "To provide sustainable services that promote good end of life care in Pembrokeshire". Paul Sartori actively empowers others, in the sector, to work towards better end of life care in all settings.
Why the community needs usPaul Sartori Hospice at Home enables people in the later stages of any life limiting illness to be cared for at home with dignity, independence, pain free and surrounded by those they hold most dear. Operating in Pembrokeshire since 1981, it offers home nursing care, equipment loan, complementary therapy, bereavement and counselling support, clinical nurse specialist, physiotherapy, advance care planning and training to support patients, families and carers. Our impact on the communityDuring 2016/2017, Paul Sartori provided: care to 1,026 people (a 25% increase on 2015/16); 19,943 care hours were provided in the community (17% increase); 74 patients and family carers received complementary therapies (23% increase); 126 patients were seen by our clinical nurse specialist; 1,147 items of equipment were loaned to 375 people; 227 patients were seen by our bereavement/counselling support team and support was given in the community by our Advance Care Planning and Training Teams.
